General Liability Waiver

1. Nature of Services

Services offered by Prismatic Flower Essences (DBA of AppWT LLC business owner Reverend Michael Allison) include Reiki, Crystal Healing, Sound Touch Therapy, Ear Seeds Consultation, Flower Essence Consultation, and related holistic and energy-healing modalities, delivered in-person at the Madison Heights, Michigan studio or virtually by video session.

These services are complementary wellness practices. They are not a substitute for medical, psychological, or psychiatric diagnosis, treatment, or care. Reverend Michael Allison is not a licensed medical doctor, registered nurse, psychologist, or psychiatrist, and no statement made during a session should be interpreted as medical advice.

2. Voluntary Participation

I voluntarily request and consent to receive these services. I acknowledge that I am participating of my own free will, that no outcome has been guaranteed, and that results vary from person to person.

3. Health Disclosure

I will inform Reverend Michael Allison of any medical conditions, allergies, prescription medications, recent injuries, surgeries, pregnancy, mental-health concerns, or other health considerations that could affect or be affected by a session. I take full responsibility for keeping this information current.

4. Assumption of Risk

I understand that holistic energy work, sound therapy, crystal placement, ear-seed application, and consumable flower essences may produce physical, emotional, or spiritual responses including but not limited to heat, tingling, deep relaxation, emotional release, fatigue, dreams, or temporary symptom intensification before improvement. I assume all risk for participating.

5. Release and Hold Harmless

I, on behalf of myself, my heirs, and my legal representatives, release, discharge, and hold harmless Prismatic Flower Essences (DBA of AppWT LLC business owner Reverend Michael Allison), Reverend Michael Allison, and any agent, employee, contractor, or licensor thereof, from any and all claims, demands, liabilities, costs, or damages arising out of or in connection with my participation in the services described above, except where caused by gross negligence or willful misconduct.

6. Confidentiality

Information shared during sessions is held in confidence and used only to support my care, except where disclosure is required by law or where I provide written consent.

7. Governing Law

This waiver is governed by the laws of the State of Michigan, without regard to its conflict-of-laws principles. Any dispute arising under this waiver will be resolved in the courts of Oakland County, Michigan.

8. Severability

If any provision of this waiver is held to be invalid or unenforceable, the remaining provisions remain in full force and effect.
